1. Number Of Monitors (indicate size – 15-17-19-21”)
2. Number Of PCs and Servers (indicate whether hard drives have been pulled and if destruction is required)
3. Number Printers, Copiers, Fax – indicate desktop, laser or large network items
4. Number and Type Of Large Units (Mini, Mainframe Type Items)
5. Location Of The Equipment (address and physical location within your facility)
6. Number of Hard Drives or other devices for destruction
